Official Glitch or Bug report form

Hey guys, this is official, certified by Alex/Blighted.

Threat Level: Minor, Medium, or Threatening. Minor would be something like a graphics or emote error. Medium would be like NPCs not being where they are suppose to be. Threatening would be like a way to dupe.
Occurs when: When the glitch occurs.
Description: Describe the bug in question.
Screenshots: This is not needed but is very helpful.
Other notes: Anything else you would want to say.
